The Clinical Documentation Improvement Program (CDI) is designed to improve the attending physician's documentation as well as improve the discharge coder's ability to code, using more accurate and complete information. The result of this should be more accurate data as well as improved reimbursement. The core of the program uses highly trained, trusted staff members to clinically review the inpatient medical record while the patient is still in-house and prompting physicians to provide clarification and detail in their documentation. This allows the record to be coded post-discharge in a timely manner in order for the hospital to be appropriately reimbursed. Physician interaction and follow-up based on clinical evidence is the primary driver of a successful CDI program. The program does not challenge the physician's
medical judgement but provides a methodology by which the physician can enhance documentation.
