Licensed Clinical Social Worker – Integrated Substance Use Disorder Treatment Program

Do you have a keen professional interest in addiction medicine and substance use treatment? California Correctional Health Care Services (CCHCS) is seeking California-licensed clinical social workers to join our Integrated Substance Use Disorder Treatment Program. This newly-created statewide program is designed to provide integrated, specialized care to patients with a history of substance use who require additional medical assistance.
Within this program and under the general direction of the Supervising Psychiatric Social Worker I, you will:
Organize and provide clinical social work services for patients with chronic diseases, including, but not limited to Substance Use Disorder, Hepatitis C, and HIV
Serve as a an adjunct member of the Primary Care Team (PCT) and collaborate with other programs/units to screen, assess, recommend level of care, determine individual treatment needs, formulate treatment goals, and plan for disposition of patients
Collaborate with internal and external partners to provide linkage of CDCR/CCHCS and community resources, facilitating patients’ effective adjustment into correctional settings and appropriate disposition from programs and/or community re-entry
