Clinical Oncology Pharmacist - Days Monday to Friday/No Weekends 5 – 8 Hour Shifts
Position Summary: The Outpatient Bone Marrow Transplant (BMT) Clinical Pharmacist Specialist provides comprehensive pharmacy services through integrated drug distribution, clinical services, research and teaching. Provides this care to Bone Marrow Transplant and Oncology patients in our clinic.

Position duties (included but not limited to):
Managerial Duties Specific to position • Oversees all aspects of formulary systems management, medication use, and drug policies for CBCI clinic (i.e. new drug approval process) • Oversees EMR build and order set maintenance • Suggests appropriate, cost-effective therapeutic medication alternatives to medical staff • Manages clinic formulary, minimizing non-formulary procurements and promotes rational drug therapy selection. • Develops pertinent policies and procedures.
Clinical Duties specific to area of focus • Provide Therapeutic Medication Monitoring of immunosuppressants, anti-infectives, anti-seizure medications, anticoagulants and chemotherapeutic agents. • Manage chemotherapy process including chemotherapy and BMT conditioning regimen order writing (paper orders) and order entry on hospital admission. • Work with IT and prescribers for OncoEMR outpatient formulary management including regimen builds, validation and conversion. • Disseminate drug information to members of the multidisciplinary team to maximize patient care. • Assist with transition of care planning including pre-transplant evaluation with medication reconciliation, drug interaction screening and patient education. • Active participation supporting committees as needed to support oncology/bone marrow transplant patient care. • Outpatient Infusion Clinic chemotherapy order entry review and compounding checks.
Basic pharmacy duties • Interpret orders and transcribes or verifies them accurately and efficiently into the pharmacy computer system. • Accurately compound and dispense pharmaceuticals. • Complies with policy and procedures and regulatory requirements. • Monitor drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies and appropriateness of drug and dose. • Provide accurate and timely medication information to healthcare staff and patients. • Assist with the antimicrobial management and other clinical programs. • Responsible for mentoring and education as well as ongoing analysis of interventions. • Participate in the training of new pharmacists, pharmacy students, and technicians. • Has knowledge of the patient population they serve and apply that knowledge in the care of those patients. • Effectively supervise pharmacy operation activities and provides support to the whole pharmacy team. • Train and participate with Code Teams as assigned. Obtain and maintain BLS-ACLS or PALS current certification as appropriate. • Maintain appropriate controlled substance records. • Comply with all regulatory standards. • Supervise and assist pharmacy technicians and other support staff effectively. • Act as supervisor in charge as assigned. • Participate in the continuous quality improvement of clinic operations (operational/clinical). • Perform other duties as assigned. • Practice and adhere to the “Code of Conduct” philosophy and “Mission and Value Statement”

A. Licensure/Certification/Registration: Current licensure by the Colorado Board of Pharmacy as a registered Pharmacist. BLS and ACLS required within first 2 months of employment. B. Education: B.S. or Doctorate of Pharmacy (Pharm.D.) C. Experience: (At least one of the following): o Minimum of 3 years of experience within area of focus. o Completion of post graduate year 2 (PGY2) oncology residency or board certified oncology pharmacist (BCOP) required. o Other certification (e.g. FCCP, FASHP).

Degree of Supervision Required: Involves general guidance and direction from supervisor. Employee is expected to perform most position duties independently and in accordance with established department and hospital policies and procedures and professional standards. Ages of Patients Served: This position requires knowledge of growth and developmental needs of patients and families. Personnel in this position must recognize and respond to the behaviors of patients and families in each age category.
