This position is responsible for collaborating with the Team Leader/ Clinical Director regarding managing, planning, organizing and facilitating the daily activities related to patient flow through the area and throughout the continuum of care. The Patient Flow Coordinator will ensure the optimal, effective and efficient flow of patients in a safe manner and provide high quality service for the emergency care center patient. Communicating patient information to provide optimum patient care in the emergency care center and supporting departments. Assists in assuming responsibility for collaborating with clinical management on Emergency Care Center throughput initiatives. Will also function in multiple roles throughout the center, as needed. The Patient Flow Coordinator is the RN who, using the nursing process, designs, coordinates and delivers care to meet the needs of the patient. The RN assesses, plans, intervenes and evaluates throughput in the Emergency Care center to provide optimal care delivery throughout the unit. Responsible for the delivery of safe, effective, and quality patient-family-centered care in the Emergency Care Center and other areas of services for all patient populations.
