Providence is calling an on call Social Worker to our Hospice Access Team in Portland, OR.
Are you an experienced social worker with a passion for hospice and excellent end-of-life care?
We are looking for an on call Social Worker that will work closely with the hospice interdisciplinary team to provide clinical counseling, assessment, crisis intervention, and psycho-education to multi-stressed patients and families relevant to the dying process, end of life care planning and bereavement. Our team understands and provides community-based care across varied residential settings while assessing for patient, caregiver, and staff safety in the home environment.
Our on call social workers are primarily scheduled in advance but may also receive some short notice requests for coverage but are not obligated to provide coverage. Hospice Access has social workers that work a variety of schedules that include days, evenings and weekends. Availability to provide support on evenings and weekends is preferred.
In this position, you will:
Under direct and continuous supervision, provide comprehensive biopsychosocial/spiritual assessment, diagnosis and/or treatment/interventions/advocacy of patients, and their support systems.
Incorporate concepts and theories associated with life span development, neurobiology, and behavioral change.
Collaborate with the patient and support system(s) to develop and implement evidence-informed person-centered care plans across the care continuum in collaboration with the Inter/Multi-Disciplinary Team (IDT).
Perform duties in a manner that respects patient self-determination that reduces health disparities/barriers to care and adheres to the National Association of Social Workers (NASW) code of ethics, Standards of Practice (SOP), and mission.
Required qualifications for this position include:
Master's degree in Social Work from a Council of Social Work Education (CSWE) accredited school of social work, or from a school in candidacy status by the CSWE
Temporary Clinical Social Work Associates (CSWA) license, or proof of CSWA initiated within 6 months of hire
Documentation of an approved supervision plan from the Oregon Board
Must obtain Licensed Clinical Social Worker (LCSW) Board licensure within a maximum of 5 years from CSWA application, or date of hire date as a CSWA
American Heart Association Basic Life Support (AHA-BLS) for Healthcare Provider certification completion within 1 month of hire
Valid driver's license and reliable automobile
Preferred qualifications for this position include:
Licensed Clinical Social Worker (LCSW)
2 years of clinical work experience in Hospice or Palliative Care
3-5 years of social work experience
2 years of working independently with vulnerable populations in a field environment
